The home health landscape is under renovation. Three-quarters of Americans prefer to age at home, but only 3.5% of US housing is considered accessible and the number of available home health aides is steadily declining. Health systems and virtual providers are establishing their own home health initiatives, primarily targeting lower-acuity care through remote patient monitoring. While monitoring plays a crucial role, it alone cannot drive meaningful change. What does matter is the action taken based on monitoring data to improve patient care and prevent costly interventions like emergency visits and hospital admissions.
