The increased demand for more individualized and convenient care is leading to a shift away from traditional provider-driven healthcare models. Aging populations, a higher incidence of chronic diseases, resource constraints, and staffing shortages are driving the move towards value-based healthcare with a greater emphasis on coordinated care. Nursing care delivery models are outlining the assignment of responsibilities, delegation of authority, and management of patient care, focusing strongly on preventive care, care coordination, and patient engagement. Nurse leaders are redesigning workflows and implementing various solutions and innovations to support these efforts, defining what the new care team looks like.
