Emory Healthcare, Georgia’s most comprehensive academic health system, has partnered with Aidin, a leading digital care transitions platform, to streamline and enhance the post-hospitalization care journey across its entire network. This collaboration marks a systemwide rollout of Aidin’s platform throughout Emory’s 10 hospitals, Emory Clinic, and affiliated post-acute care providers, with the aim of improving care coordination, accelerating patient discharges, and delivering better patient outcomes.
By integrating Aidin’s platform, Emory will optimize how patients are placed into post-acute care facilities by using real-time data to match individuals with providers based on location, availability, and insurance coverage. This approach is expected to shorten hospital stays, reduce administrative workload, and enhance the overall efficiency of discharge planning. Aidin’s tools also support patients, families, and caregivers in making more informed decisions, empowering them to select post-acute care providers that best meet their needs.
