Home care is a rapidly growing industry in healthcare, involving an increasing number of patients, health plans, and providers. However, this form of care is often mismanaged and uncoordinated, resulting in poorer quality and higher costs. Plans and providers face significant administrative burdens and high costs associated with coordinating across complex networks of stakeholders.
